Sexual Ethics Exemption Request form:   For clergy no longer performing any ministerial functions currently or in the future.

Sexual Ethics Deferment Request form: For clergy on medical leave who are performing no ministerial functions or related activities and are unable to take the test due to the nature of their illness.
